Welcome to CGM Charlotte! The terms “we”, “us” and “our” refer to CGM Charlotte. CGM Charlotte operates this website to provide general information about continuous glucose monitoring (“CGM”) and to allow visitors to contact us or request help getting set up with CGM supplies (the “Services”). This website is not an online store and does not accept orders or process payments through online checkout.
If you submit your information through our Services, we may contact you by phone and/or email to help determine eligibility, verify insurance coverage, collect necessary details, and assist with CGM setup through available insurance coverage (including Medicare, UnitedHealthcare, Aetna, BCBSNC, and BCBSSC, among others). If approved and appropriate, we may arrange to supply CGM equipment and ongoing supplies through the applicable process outside of this website.
CGM Charlotte is powered by Shopify, which enables us to host and operate the Services.
The below terms and conditions, together with any policies referenced herein (these “Terms of Service” or “Terms”) describe your rights and responsibilities when you use the Services.

By visiting, interacting with, or using our Services, you agree to be bound by these Terms of Service and our Privacy Policy. If you do not agree to these Terms of Service or Privacy Policy, you should not use or access our Services.

By agreeing to these Terms of Service, you represent that you are at least the age of majority in your state or province of residence, and you have given us your consent to allow any of your minor dependents to use the Services on devices you own, purchase or manage.
To use the Services, you may be asked to provide certain information such as your name, phone number, email address, address, date of birth, and insurance information so we can contact you and assist with CGM setup. You represent and warrant that all the information you provide through the Services is correct, current, and complete and that you have all rights necessary to provide this information.
You understand and agree that CGM Charlotte may contact you by phone call, voicemail, text message, and/or email using the information you provide in order to respond to your request and help coordinate CGM setup. Message and data rates may apply for texts, if used.
CGM Charlotte does not accept online orders or online checkout through this website. Do not submit payment card information through the Services.
You are solely responsible for maintaining the security of your account credentials and for all of your account activity. You may not transfer, sell, assign, or license your account to any other person.
SECTION 2 - INFORMATION ABOUT PRODUCTS AND SERVICES
We have made efforts to provide an accurate representation of CGM-related products and services on the Services. However, information such as product features, availability, coverage criteria, and other details may change and may vary by insurer, plan, and individual eligibility.
Any product references on the Services are for informational purposes only and are not offers to sell products through online checkout. Product availability, eligibility, and fulfillment are subject to applicable requirements, including medical necessity, documentation, and insurance approval where applicable.
SECTION 3 - REQUESTS FOR SERVICE; NO ONLINE CHECKOUT
When you submit a form or request through the Services, you are requesting that CGM Charlotte contact you and assist with CGM setup. Submitting a request does not create an online order or a binding agreement to purchase, and CGM Charlotte does not accept orders through website checkout.
After you submit your request, we may contact you by phone and/or email to obtain additional information and help determine eligibility (including insurance verification and any documentation requirements). If you are approved and eligible, CGM Charlotte may arrange to supply CGM equipment and/or supplies through the appropriate process outside of this website.
We reserve the right to decline or discontinue assistance in our discretion, including if we are unable to verify information, if eligibility requirements are not met, if required documentation is not provided, or if coverage is unavailable.
SECTION 4 - PRICES, COVERAGE, AND BILLING
CGM Charlotte does not process payments through online checkout on this website. Coverage, copays, deductibles, coinsurance, and out-of-pocket costs (if any) depend on your insurance plan and eligibility. Any cost estimates discussed are not guarantees and may change based on insurer determinations and updated plan information.
If payment is required for any portion of CGM supplies, it will be handled through the appropriate billing process and consistent with applicable requirements. Do not submit payment card information through the Services unless specifically requested through a secure, authorized payment method outside of this website.
SECTION 5 - SHIPPING AND DELIVERY
Because purchases are not made through online checkout on this website, shipping and delivery are not initiated through the Services. If CGM supplies are approved and arranged, shipping and delivery timeframes (if applicable) will be communicated to you separately and may be subject to carrier delays or events outside our control.
SECTION 6 - INTELLECTUAL PROPERTY
Our Services, including but not limited to all trademarks, brands, text, displays, images, graphics, video, and audio, and the design, selection, and arrangement thereof, are owned by CGM Charlotte, its affiliates or licensors and are protected by U.S. and foreign patent, copyright and other intellectual property laws.
These Terms permit you to use the Services for your personal, non-commercial use only. You must not reproduce, distribute, modify, create derivative works of, publicly display, publicly perform, republish, download, store, or transmit any of the material on the Services without our prior written consent. You may be provided with access to customer tools offered by third parties as part of the Services, which we neither monitor nor have any control nor input.
You acknowledge and agree that we provide access to such tools “as is” and “as available” without any warranties, representations or conditions of any kind and without any endorsement. We shall have no liability whatsoever arising from or relating to your use of optional third-party tools.
Any use by you of the optional tools offered through the site is entirely at your own risk and discretion and you should ensure that you are familiar with and approve of the terms on which tools are provided by the relevant third-party provider(s).
We may also, in the future, offer new features through the Services (including the release of new tools and resources). Such new features shall also be deemed part of the Services and are subject to these Terms of Service.

CGM Charlotte is powered by Shopify, which enables us to host and operate the Services. However, cgmcharlotte.com does not facilitate online sales or purchases through the Services. By using the Services, you acknowledge and agree that Shopify is not responsible for any aspect of interactions between you and CGM Charlotte, including any outcomes related to information provided through the Services or communications initiated through the website.
You hereby expressly release Shopify and its affiliates from all claims, damages, and liabilities arising from or related to your use of the Services.
SECTION 10 - PRIVACY POLICY
All personal information we collect through the Services is subject to our Privacy Policy, and certain personal information may be subject to Shopify’s Privacy Policy, which can be viewed here. By using the Services, you acknowledge that you have read these privacy policies.

The following sections will continue to apply following any termination: Intellectual Property, Feedback, Termination, Disclaimer of Warranties, Limitation of Liability, Indemnification, Severability, Waiver; Entire Agreement, Assignment, Governing Law, Privacy Policy, and any other provisions that by their nature should survive termination.
